The BVP Foundation works tirelessly
to make sure that our specified target populations are serviced adequately.
Some of our programs include focus groups for HIV/AIDS awareness and prevention led by Medical
Doctors and Registered Nurses. We offer health awareness seminars, mentor-ship, as well as internship programs.
NEW IN 2009: GRANT
FOR QUALIFYING RECIPIENTS
Each quarter, qualifying individuals are chosen to receive Full lace units/ lace front units
free of charge that are suffering from alopecia, hair loss due to cancer chemotherapy treatments, or hair loss due to side
effects of medication. Beginning in May 2009, some individuals may qualify for grants from the AMERICAN CANCER SOCIETY
& THE ALOPECIA AREATA FOUNDATION.

If an individual requires a "cranial hair prosthesis" while undergoing Alopecia or cancer-related therapy,
they may be eligible for reimbursement under their health insurance plan?
Most insurance companies cover between 80% to 100% of a cranial prosthesis.
If a wig needs to be worn related to medical problems, the wig should be called a "cranial prosthesis"
for insurance purposes.
Follow these guidelines below to
assure coverage, when eligible through your insurance company.
1.
Obtain referral, prescription, and ICD-9 (diagnosis code) from your physician
2. BeautifEYED Vision Products will prepare an invoice for "cranial hair prosthesis"
3. Follow the guidelines for filing your insurance claim
4. Check the benefits and eligibility guidelines of your insurance. There
should be a section under "covered expenses" relating to prostheses, prosthetic devices, or medical durable equipment.
5. Make sure to complete insurance form and obtain signature from your physician.
Ensure that your physician writes a prescription for a "full cranial prosthesis" (not a wig) and documentation explaining
hair loss and its effects. Include emotional well-being in relation to cosmetic.
6. Keep copies of your sales or invoices for your cranial prosthesis. (portions of unpaid items by insurance
are tax deductible).
7. You can also include letters
from your employer, pictures of yourself without hair, & a hand written letter detailing the emotional effects your condition
has had on your life
IF YOU ARE DENIED
1. Ask for review by a medical review board,
include the original claim
2.
Ask for written reply as to why claim was denied
3. Send letter to claims department supervisor
